     All correspondence including purchase orders, payments, etc. may be sent to P.O. Box 208, Layton, Utah 84041.  The United States Industrial Supply warrants to the original purchaser that its new products manufactured by United States Industrial Supply will be free from defects in material and workmanship for a period of 3 months from the date of the original shipment to end user.  Any new product manufactured by United States Industrial Supply found to be defective in material or workmanship within the warranty period will be repaired or replaced ( at United States Industrial Supply's option) without charge to the original purchaser when returned with proof of purchase to United States Industrial Supply with transportation prepaid and insured.  When ordering, be sure to fill in all information correctly, so that if any problems occur they can be cleared up before the order clears processing and is shipped to you. Items can be returned, the customer will be responsible to pay the freight charges both ways, along with a 15 % restocking charge. There will be a $35 fee on credit card charge-backs that are proven incorrect. If an item is defective the manufacturer of the item will warranty the item, not United States Industrial Supply. All items manufactured by United States Industrial Supply will be warranted by United States Industrial Supply.  Many items we carry are special order items so be sure to order the items that you want.  Thanks from the sales staff at United States Industrial Supply. All implied warranties, including any implied warranty of merchantability or fitness for a particular purpose, shall be limited in duration to the 3 - month express warranty period specified above.  This warranty is void if: (1) if product has been damaged by accident or unreasonable use, neglect, improper service or installation, or other causes not arising out of defects in material or workmanship.  All returned goods require an R_A_# issued by United States Industrial Supply.  Claims for shortages or errors must be made within 10 days of receipt.  Freight damages or shortages must be noted when merchandise is received, and claim filed with the freight carrier.   Send returned goods to United States Industrial Supply  965 North Redwood Road, North Salt Lake, Utah 84054 .
